Pupil Personal Accident Insurance

All pupils in our school are covered by the insurance which we take out with Allianz each year. This 24 hour cover relates to school activities and social, domestic and leisure activities.

If your child is injured (accidental bodily injury) and there are costs relating to the injury you are entitled to make a claim.

Please Note: You can only claim from one insurer.

Procedure for Making a Claim

The parent/guardian completes the claim form available below:

The claim form should be signed by the parent/guardian and will also need to be signed by the school Principal.

The claim form should then be sent to Allianz along with any accompanying medical/dental receipts. (If there are likely to be additional expenses as a result of the accident, please send a note with the claim stating that and Allianz will keep the claim open).
